This Form is designed to help make a complaint to the Office of the Data Protection Commissioner. Your rights under the Data Protection Acts 1988 and 2003 are set out here.

Completion of this form does not automatically mean that an Investigation will be commenced. The onus is on you to provide this office with full details of the issue which you are raising. Adequate supporting documentation needs to be supplied so that the issue can be properly assessed. Ensuring that you provide full details, at this point, will enable this office to process your submission more efficiently, in turn reducing response times for you.

Before you submit a complaint to the Office of the Data Protection Commissioner you should contact the organisation concerned in an attempt to resolve the complaint in the first instance.

Under the Acts, information which an organisation holds about you must be accurate, only made available to those who should have it, and be used for the specific purpose for which it was gathered.

You also have the right to: access information relating to you, (in some cases) have it removed and have any factual errors rectified.

You may be receiving unsolicited marketing in the form of electronic communication. This marketing may be sent by(but is not limited to) text or e-mail.
