I am a freelance consultant state registered Dietitian, registered with CORU and a member of INDI. I run a Dietitian clinic in Castlebar, Co. Mayo. Patients are referred to me by GPs and consultants. I also accept self referrals. Patient data which is recorded is kept securely on site. I shred paper records and dispose of them appropriately.

I use personal data to be able to undertake a dietetic and nutrition assessment, correspond with other health professionals in relation to the clients medical care and my dietary plan and recommendations for them. Name, address, date of birth, contact number, email address, GP and/or consultants name, diagnoses, dietary plan, and any referral letter received by me from a referring GP or Consultant . |

Patient information may be disclosed to the patient on request, I share letters and records with patients on request and often share their discharge letter to their referring GP or consultant. GP, Consultant and Patient themselves. Verbal consent is provided from the patient/client and documented in my notes prior to sending a letter to their referring GP and/or consultant. |
