Using personal details- as mentioned earlier plus PPSN/ medical card number/insurance company details for processing bloods/swabs/smears, referral to specialists/ public health nurses, palliative teams, to generate prescriptions, to fill out the forms, like diving applications, social welfare certs and documents, medico-legal and insurance forms, etc. Name, DOB, Address, contact details- phone numbers, PPSN, Medical card details, next of kin contact details, Insurance company details; Personal history, past medical history, social history, history of family conditions, information received from other health care professionals (discharge letters, etc), results of tests/investigations/ examination performed etc- routinely done while attending a GP surgery. |
