Treatment undergone prior to attendance, occupation, email address, is there legal case in relation to injury, referred by General Health, hospitalisation, investigations to date - MRI xrays, medication, know conditions - pregnancy, diabetes, epilepsy, heart condition, pacemaker, chest condition, any know allergies, other medical condition diagnosed, history of night pain, unexplained weight loss, signature of patient and date of registration. |
