| Purpose: | (a) We are a general medical practice. We record details of patient names, addresses, dates of birth, telephone numbers, medical card numbers and details of our encounters with patients. We record information supplied to us by the patient, patient representatives, hospital and other outside agencies (b) We write to outside agencies, such as hospital, social welfare, the HSE and insurance companies on our patients behalf (c) We use the information we have recorded to perform audits of our clinical practice, we code diagnoses to ensure that at risk groups can be identified (d) Vaccines as per the national immunisation guidelines are delivered e) return of health data to the HSE as part of the under 6 scheme and diabetes cycle of care |

| Description: | (a) Personal data is used to ensure we record information to the correct file, to ensure that we can contact the patient in an urgent situation and to ensure that we have a ocmprehnesive medical record - This requires a number of patient identifiers such as name, date of birth, address, GMS number and phone number. (b) We need to refer patients to hospitals and their staff, and this requires a referral letter to be done. Medical card applications often need a background medical history and list of current medications to see if the patient is eligible for the card. Insurance companies write to us, with the patients consent, for a summary of medical history and clarification of ongoing issues - This requires a number of patient identifiers such as name, date of birth, address, GMS number and phone number. It also requires medical information to be disclosed. (c) Audit uses our patient data base, of name, gender, date of birth, medication, disease and vaccination to profile our population and identify areas where care can be improved - This uses our patient data base, of name, gender, date of birth, medication, disease and vaccination history. (d) The HSE has a national immunisation schedule, both for children and adults - The childrens form details the childs name, address, date of birth and the parents/ guardians signature. It also list the vaccine type, batch number, and date of administration. Adult vaccine returns have the GMS number and name. (e)The HSE requires us to return information on heights and weights of children under 6 and on health status of patients with type 2 diabetes |

Insurance companies write to us, with the patients consent, for a summary of medical history and clarification of ongoing issues - Hospital staff, nurses, doctors, laboratory staff, medical administration will have access to information in referral letters we send to the hospitals. HSE staff will see our letters to the HSE. Insurance company staff will have access to the health records we return, but there are returned in the envelopes for the attention of the medical referrees (c) Audit uses our patient data base, of name, gender, date of birth, medication, disease and vaccination to profile our population and identify areas where care can be improved - The medical staff of the surgery will have access to the unique patient identifiers for audit purposes and patient recall. No raw data that identifies patients directly will be disclosed to outside agencies, but the overall audit results may be required in the future for competence assurance by the medical council and HSE (d) The HSE has a national immunisation schedule, both for children and adults - The HSE staff will have access to this information |
